I am not sure what the name of this restaurant was, but the address is "2251 S. Wentworth Ave., Chicago, Illinois. This is from Chinatown in Chicago. This is a folding fan. It has wooden slats that are held together at the end with a metal loop. The paper design holds the slats together. This fan has hand painted flowers. When open it is 14" wide point to point. It is 8-1/2" tall. It has an ink stamp mark of "Japan" on one end wooden piece on the back.
This fan does have some small tears mainly along the creases. The longest is 3/4" long above the second symbol in the brackets. The left side is a bit scrunched. This needs to be glued and flattened out.
