This is one tiny tiny Perfume Corkscrew....also used during the 1800's for those drug & pill bottles that used only small corks!!
Originally perfume was sold in small bottles that had cork stoppers, and you needed a very small corkscrew to remove them....this one is only 1-1/16" tall x 1.5" wide at the handle.
The handle is silver plated, and the screw part is in steel. It probably was part of an elaborate Dresser Set at one time, from the late Victorian period. The decor is a basket of flowers and leaves.
