It has a soft blue cover with a black and white print with ads for Dr. Bull Cough Syrup on inside and back cover. Cannot find dating. It has good thoughts messages throughout and encourages abstaining from drinking except for Dr. Bull's Cough Syrup ! The measurement of this booklet is almost 5 1/2" long x 4" wide. It has a issues on condition such as torn corners, bent corners, etc. but is still intact and probably more important, that is survived all these years!
